Ms. L was 36 years old with an AMH of 0.68 ng/mL. In her first cycle elsewhere: 8 eggs retrieved; after PGT, 0 transferable embryos. A few months later she completed a second IVF cycle at Dan Ruo IVF Clinic (DRC) in Bangkok: 19 eggs retrieved, and after testing she obtained 6 transferable blastocysts.
AMH is a blood marker related to ovarian reserve. It roughly reflects how many follicles may still be recruitable; it does not equal egg quality or guarantee pregnancy. Age mainly affects egg quality and aneuploidy risk. At 36 with AMH 0.68, clinicians often view this as diminished ovarian reserve—quantity pressure plus age-related quality pressure—so one cycle’s yield is hard to promise in advance.
The two cycles unfolded as follows.
First cycle: another clinic
Egg retrieval in January 2026:
- 8 eggs retrieved, 4 mature
- 2 blastocysts cultured
- After PGT: 0 transferable
After injections, blood draws, and waiting, a report showing “0” is hard to face. She rested for a period, then about three months ago came to Dan Ruo IVF Clinic for consultation and report review, and started the second cycle.
Second cycle: Bangkok IVF (about 10 days of stimulation)
From 16 to 25 August 2026 she underwent about 10 days of ovarian stimulation at Dan Ruo in Bangkok (Gonal-F, medroxyprogesterone, and clomiphene), finishing with a dual trigger (triptorelin + Ovitrelle).
On 23 August, monitoring showed about 24 follicles across both ovaries:
Final retrieval: 19 eggs:
- Mature (MII): 12
- Immature MI: 2
- Earlier immature GV: 5
ICSI was performed mainly on the 12 mature eggs. At the fertilization check, all 12 showed normal 2PN:
On Day 3, all 12 embryos were still developing:
- 1 × 10-cell grade 1
- 5 × 8-cell grade 1
- 6 × 6-cell grade 1
Blastocyst cryopreservation from Day 5:
- Day 5: 4 (5AA / 4AB / 4BB / 4BB)
- Day 6: 3 (5AA / 4BB / 5AB)
- Day 7: 2 (3BC / 3BC)
After blastocyst biopsy, embryos were sent for PGT (preimplantation chromosomal screening, often discussed with third-generation IVF pathways). Eight blastocysts were tested; the report showed 6 transferable:
- Day 5: 4AB, 4BB, 4BB
- Day 6: 5AA, 4BB, 5AB
- 2 others: one with chromosome 18 abnormality; one with complex abnormalities
What this meant for Ms. L: with 6 screened transferable blastocysts, uterine evaluation and transfer planning could proceed with less fear of having nothing left to transfer.
Two-cycle summary
Elsewhere: 8 eggs → 0 transferable. Bangkok: 19 eggs → 6 transferable. The second cycle in brief:
1. Larger egg yield
8 elsewhere vs 19 in Bangkok.
2. No loss at fertilization of mature eggs
12 MII eggs via ICSI; all 12 normal 2PN.
3. Relatively even Day-3 development
All 12 were still growing with similar cell numbers and grades.
4. Blastocyst culture and screening kept up
8 tested; 6 transferable.
This was not simply “a different drug turned 0 into 6,” but the stack of yield, fertilization, culture, and screening.
